My First Time Clients
Your initial appointment will take approximately 15 minutes extra to fill out your paperwork, discuss your health conditions and allow time to answer all questions before and after your massage. While discussing your massage you can decide which type of massage you would like and/or choose the integrated massage, where I will use a variety of techniques designed to address your specific needs.
After our discussion, you will be taken to the massage room. Massage time starts from the time the massage actually starts.
When scheduling, I try not to schedule clients back-to-back, so that there is adequate time to devote to each client. This allows time to answer all of your questions either before, or after your massage.
If you would like to save time at your first appointment, you can print the client form and bring it filled out to your first appointment.
Regular Client Form
Prenatal Client Forms
The following form is for your doctor/midwife to fill out if your pregnancy is considered high-risk. Please take it to your next prenatal appointment and discuss it with your caregiver. Prenatal massage should not cause you any problems, only bring you relief, however for your comfort and mine, it is nice to have your doctor on board with your choices.

It is used as a complementary therapy for the treatment of physical, emotional, and mental diseases. Tuning Fork Therapy Tuning forks come from the field of Sound Therapy. They can bring the body back into harmony using vibration. Much like singing can make us feel better, tuning forks work more specifically by vibrating each chakra and bringing it into perfect harmony.
There are tuning fork for each chakra of the body. Each chakra has a unique frequency and each chakra has a specific tuning fork set to that perfect frequency. These forks dissolve negative energy and bring us back into balance. Two different kinds of tuning forks are used: Weighted and Unweighted. Weighted are excellent for reducing pain, reducing inflammation and inducing relaxation. Unweighted are excellent for mental, spiritual and emotional balancing, improving energy fields, improving physical health, and helping to create inner harmony and wholeness
